Annual Flu Vaccinations
We are currently in the midst of that dreaded flu season and although the Court has already held its annual Flu Vaccine Clinics in several of the Court locations, if you were unable to obatin your vaccine, or if you would like for your enrolled dependents to obtain a vaccine, you may stop into any participating pharmacy to receive your vaccine and submit it for reimbursement from the Court's Health Plan. Flu Vaccination Reimbursement Claim Forms may be recieved by contacting Kristin Bush in the Human Resources Department. Please note that in order for a reimbursement to be made under the Flu Vaccination Reimbursement program, all recipients must be enrolled in active benefits through the Court's Health Plan, must not have already received a vaccination for the same year, and must include the claim form and back-up documentation showing the recipients name, date of vaccination and proof of payment.
